  Former Student Record and Diploma Request Forms

To obtain former student record(s) please click on the Student Request Form link below. You may either mail the the completed request form and fee to the address on the top of the form or bring the form into the office.

All 3rd Party Requestors must request documents or Verification of graduation online. Please click "Go to Online Student Record Request" below and follow the instructions

To expedite the transaction, you may fax your completed form to our office prior to your arrival.

Requests will be completed within 24 hours, unless documents cannot be found. The Records Management Center will contact the individual requesting the document(s) if information cannot be found.

NOTICE:  As of June 20, 2012 Regulation 790-2:

The Prince William County Schools Records Center issues a $5.00 per page fee for any document(s) requested for personal use. To expedite the request a cash or money order must accompany the request. Personal checks will not be accepted and will be returned to the requester. Please contact our office for the total fee amount if there is more than one document requested.
 


To obtain a copy of a former graduate student's diploma from Prince William County Schools, click on the Diploma Request Form link below. You may either mail the the completed request form and fee to the address on the top of the form or bring the form into the office.

Requests are sent to the HERFF JONES company and take six to eight weeks to process. 

The fee for a duplicate diploma is $35.00 and should be made payable via MONEY ORDER only to HERFF JONES. NO EXCEPTIONS.  


GED

Beginning January 2, 2014 all GED Certificates/Transcripts dated 2002-present date shall be ordered through the GED Credentialing web site.  Records dated prior to 2002 may be ordered online or on a walk-in basis.

The release of student information requires a signed request from the student or the parent / legal guardian if the student is under 18 years of age except in specific, limited conditions. If you have not previously given written permission to release information, a signed request can be faxed or mailed. Note:  Requesting the Records Management Center to fax a document can only be completed if the document has not been microfilmed and the fee has been paid by cash or money order.
